Anonymous Incident Report Form

If you feel wronged or feel you have witnessed wrongdoing, we want to hear about it. We welcome all opportunities, big and small, to provide education on our community’s expectations so that we can maintain a safer space for all. Reporting issues also help us monitor and address larger patterns of behavior in individuals and in the community.

Unless otherwise requested, your report will remain anonymous. If you request to be contacted about the incident, we will not use your contact information for any other purposes.

Describe the incident:

Please provide a detailed description of the incident, including where it happened, the sequence of events, anything that happened as a result, and the names of individuals who were involved in or witnessed the incident. Names will remain confidential unless the individual gives consent for their name to be shared or if there is immediate danger to any named individuals.

Next Steps

As a result of your report, a Dancing Fools staff member will contact the individual(s) involved in the incident to discuss their behavior and expectations for their conduct going forward. If they acknowledge their infraction and correct their behavior immediately, the incident will be considered resolved. If additional action is required (eg. a restorative circle) after a staff member contacts the individual(s), we want you to be involved as much or as little as you feel comfortable. You may also request that the Dancing Fools do not get involved beyond the contact described above. Separately, we can let you know the outcome of your report upon request.
